Do interior designers create a look then have the furniture/decor/etc. manufactured or...?

Do interior designers create a look then have the furniture/decor/etc. manufactured or do they shop for items all over and create a look that way? Let's say an interior designer is designing a bathroom, do they illustrate new facets, sink, tub, racks, toilet, etc. and then all of those new designs are manufactured for a client, or do they look at the bathroom and choose certain items for it based on what's available in stores? Thanks

It's not unusual for designers to have furniture custom built, but in the case of plumbing fixtures, it would be extremely cost prohibitive to have such things fabricated, especially when there are literally thousands to choose from in the "to the trade" market. Same for furniture really, but sometimes the space requires it more than say a bathroom would. Bottom line, what a designer chooses is all about the client's budget. Sometimes we choose off the shelf big box items & sometimes the space & client requires all custom.
