how to apply concrete overlay to countertops?

I saw on HGTV someone applied a concrete overlay on a bathroom counter. I want to do that on my kitchen counter tops, but I need to know what products to use.

The numerous designs that can be cast and the variation in colors make it nice for fitting into any kitchen style. I hope the videos help.
