What are the average measurements from top cabinets to countertop in kitchens?

How far should they come out from the wall and how much space from the counter are the average cabinets placed?

1. 12 inches out from the wall, 18 inches from the top to bottom of cabinets. http://www.woodbin.com/ref/furniture/cabinets.htm
2. most states have building codes for this. The international code book says it should be 18 inches from the top of the countertop to the bottom of the cabinet. wall cabinets come out 12 inches from the wall with a few exceptions. and base cabinets are 24 inches from the wall. countertop is usually 1.5 inches thick and 25 inches from the wall.
